Above are details of my wife Marcia Lyons dob 6/10/49. At check in the agent refused her documents for her service dog. I previously detailed the information in another post. Date of details of mental health professional was Dec 2017 and agent said it was a year old. Wife tried to connect agent with shrink who still would not permit wife to travel. Forced to take uber to residence where there wa no food and uber next morning when the station manager who we knew permitted her to fly. We want this rude, obnoxious agent retrained. No reason she should ask my wife what her condition is requiring the dog. This would violate HIPPA. We have flown Allegiant even before AVL opened and we were on inaugural flight.

Reason of review: Poor customer service.

I didn't like: Way they treated my wife on jan 10, Check in agent who was rude.

All the information and requirements for traveling with a service animal is posted clearly online.It says letter must be on Dr letterhead dated within a year from flight.Though the agent shouldn't be rude about it.
